If you are seeking an ADHD evaluation, please download and complete this form and have your child's teacher(s) complete the Teacher form (below) prior to your child's appointment. If you have not scheduled this visit, please call our office. We schedule these visits at the end of the day and thus we are limited in how many we schedule each week.

In addition to completing the Parent form, please have your child's teacher(s) complete this Teacher form prior to your child's ADHD evaluation so that our physicians may review it with you during your child’s appointment.
